Our Proactive, Comprehensive Care Model to Enhancing Your Life.

At the heart of Sterling Estates’ approach to resident care is the goal of enhancing residents’ quality of life and significantly reducing preventable emergency room visits and hospitalizations; more commonly known in the industry as “Value-base care”. We call our approach, the “Sterling Care”. Our Sterling Care is designed to be proactive and simplify the often-complicated process of caring for a loved one. By collaborating closely with families and healthcare providers, we aim to improve the overall health and wellness of our residents, providing a stark contrast to the challenges of coordinating care independently.

Specifically, we contract with an independent on-site care provider, Transitional Care Physicians (TCPA), consisting of gerontology physicians and nurses who see our patients regularly and as needed. Additionally, we partner with Guardian Pharmacy of Atlanta, which offers cost savings and qualitative efficiencies to our residents. These relationships provide significant value to our residents’ health and well-being across all segments of living in our communities.

Sterling Care: Our Approach

Each Sterling Estates community features a dedicated Resident Care Director who actively assists residents in managing their daily health needs.

To ensure the highest quality of care, our team partners with other healthcare professionals to support our residents during care transitions, ensuring seamless communication between providers, reconciling medications, and scheduling follow-up visits with physicians.

In Assisted Living or Memory Care, the Care Director creates and implements individualized care plans.

For Independent Living, the Care Director and team provide support and advocacy.

How Sterling Care Helps Close Gaps in Care:

Step 1: Every Sterling community benefits from a Resident Care Director who is a registered nurse/LPN.

Step 2: We support our Resident Care Directors with up-to-date technology and communication tools to enable responsive, effective care coordination with our residents’ primary care providers and other specialists.

Step 3: The Resident Care Director meets with every new resident to understand their health condition and create a personalized care plan. This plan is based on their healthcare needs, values, goals, and preferences. During this meeting, the resident also decides if they want to receive primary care services on-site or outside the Sterling community, with input from their primary care team.

Step 4: The Resident Care Director monitors the overall health and wellness of the residents and helps triage urgent care needs.

Step 5: When residents need additional healthcare services, the Resident Care Director organizes care transitions, coordinates referrals, reconciles medications, and schedules follow-up visits.
