Sterling Estates’ care coordination program is designed to simplify the often-complicated process of caring for a loved one, allowing us to focus on providing an improved level of care for our residents. By collaborating with families and healthcare providers, we aim to enhance residents’ quality of life and reduce preventable emergency room visits and hospitalizations.

Each Sterling Estates community features a dedicated RN Care Director who actively assists residents in managing their daily health needs. This RN Care Director partners with other healthcare professionals to advocate for residents during care transitions, ensuring seamless communication between providers, reconciling medications, and scheduling follow-up visits with physicians. Our comprehensive approach ensures residents receive the highest level of coordinated care and support.

How Sterling HealthPlus Helps Close Gaps in Care:

Step 1: Every Sterling HealthPlus community benefits from a care manager who is a registered nurse.

Step 2: We support our care managers with up-to-date technology and communication tools to enable responsive, effective care coordination with our residents’ primary care providers and other specialists.

Step 3: The RN Care Manager meets with every new resident to establish their current health status, then develops a personalized care plan based on their healthcare needs as well as individual values, goals and wishes. This meeting also confirms whether a resident prefers to receive primary care services on-site or outside their Sterling community – with input from the resident’s primary care team.

Step 4: The RN Care Manager monitors the overall health and wellness of the residents and helps triage urgent care needs.

Step 5: When residents need additional healthcare services, the Sterling HealthPlus RN Care Manager organizes care transitions, coordinates referrals, reconciles medications, and schedules follow-up visits.
